    • Original/Certified Copy of Birth Certificate
    • Current year School ID
  • Proof of physical address, if not listed on ID (verification acceptable via student's M-DCS portal)
  • Students ages 14 and under must be accompanied by a parent or guardian2

1 If you have not made a withdrawal from, deposit to, or transfer involving your account for more than one (1) year, the Credit Union may classify your account as a dormant account. The Credit Union imposes a monthly service fee for continuing to process your dormant account as set forth on the Service Fee Schedule.

2 Parent/legal guardian must be joint on the account and must provide a valid form of ID with current address.
