- US driver's license

- Original/Certified Copy of Birth Certificate
- Current year School ID
- Proof of physical address, if not listed on ID (e.g., Educational institution transcript forms for the current school year or proof of physical address in the parent's name)
- Minors ages 12 and under must be accompanied by a parent or guardian1
